Start with a sharp line drawing ...
Paint a "depth map" that describes how far lines and shapes are away from the viewer. Lighter shades mean closer.
Through a software called Depth of Field Generator Pro, add a sense of depth to the drawing by applying the depth map to blur lines and shapes.
